DOG TRAINING CLIENT REGISTRATION & AGREEMENT FORM

I understand that dog training results cannot be guaranteed and that progress depends on consistent practice and appropriate management outside of training sessions.

I understand that I am responsible for the safety, welfare and control of my dog at all times while attending training sessions. I agree to follow the trainer’s instructions and any reasonable safety measures given during the session.

I confirm that I will provide accurate and complete information about my dog’s health, behaviour and history, including any history of growling, snapping, lunging, biting, aggression, fear, anxiety or reactivity towards people or dogs.

I agree to inform the trainer of any relevant changes to my dog’s health or behaviour before attending a session.

I understand that training advice is not a substitute for veterinary care and that I may be advised to seek veterinary assessment where appropriate.

I understand that participation in group training is undertaken at my own risk and that dogs may be exposed to unfamiliar people, dogs, environments and other distractions during sessions.

I understand that dogs will not be permitted to interact freely with other dogs during class unless specifically instructed by the trainer and that I must follow the trainer’s instructions regarding distance, handling and positioning.

I understand that the trainer may modify, pause or stop an exercise if they believe it is necessary for the safety or welfare of my dog, another dog, another person or the trainer.

I understand that the trainer may ask me to leave a session, or recommend alternative training arrangements, if my dog’s behaviour presents an unacceptable safety risk or if group training is not considered appropriate for my dog.

I agree to use appropriate equipment and to follow any equipment or handling instructions provided by the trainer.

Photography & Marketing Consent

I consent to photographs and/or videos of myself and/or my dog being taken during training sessions and understand that these may be used for training records, social media, website content and other marketing purposes. (tick box below)

Data Protection

Your information will be used for training, communication, administration and record-keeping purposes.

Your information will be kept securely and will not be shared with third parties unless required by law or where necessary to protect the health, safety or welfare of a person or animal.

Your information will be retained only for as long as reasonably necessary for these purposes.
